In March 2020, a federal grand jury indicted a California physician for unlawfully distributing opioids, including fentanyl, outside the level of his professional practice. Physician Timothy Mulligan ran a practice in San Mateo where a substantial portion of his practice involved dispensing opioid prescriptions. The investigation determined Mulligan handed out an abnormally high amount of full-strength opioid prescriptions over the years.

According to California state prescription records, Mulligan handed out over 9,000 fentanyl, oxycodone, and hydrocodone prescriptions to more than 250 patients between August 2014 to June 2018. Investigations also discovered that Mulligan processed private and Medi-Cal patient insurances to cover office visits and payment for the unnecessary amounts of drugs. He was additionally charged with federal health care fraud in the state of California.
