What Is a DOT Drug Test?

A DOT drug test is a federally mandated urine drug screening required for employees performing safety-sensitive duties in DOT-regulated industries. Unlike non-DOT drug tests, DOT testing must follow strict federal guidelines outlined in 49 CFR Part 40, including:

  • Regulated urine collection procedures
  • Official DOT Chain of Custody Forms (CCF)
  • Testing at HHS-certified laboratories
  • Mandatory review by a licensed Medical Review Officer (MRO)
  • Secure and compliant result reporting

DOT drug tests are legally defensible and designed to protect public safety across the transportation industry.

Industries & Roles That Require DOT Drug Testing

DOT drug testing applies to employees who perform safety-sensitive functions in transportation-related industries regulated by the U.S. Department of Transportation. These roles directly impact public safety, which is why federal law requires strict drug testing protocols before hiring and throughout employment.

Health Street supports DOT drug testing across all major DOT agencies, including:

FMCSA: Trucking & CDL Drivers

The Federal Motor Carrier Safety Administration (FMCSA) requires DOT drug testing for commercial motor vehicle (CMV) drivers who hold a Commercial Driver's License (CDL). This includes drivers operating vehicles used for interstate commerce, passenger transport, or hauling hazardous materials. Testing is required for pre-employment, random selection, post-accident situations, reasonable suspicion, and return-to-duty.

FAA: Aviation Personnel

The Federal Aviation Administration (FAA) mandates DOT drug testing for safety-sensitive aviation employees, including pilots, flight crew members, aircraft mechanics, dispatchers, and ground crew personnel who perform critical operational duties. DOT compliance in aviation helps ensure the safety of passengers, crew, and aircraft operations.

FRA: Railroad Workers

The Federal Railroad Administration (FRA) requires DOT drug testing for railroad employees who perform functions that impact train operations and rail safety. This includes locomotive engineers, conductors, signal operators, dispatchers, and other rail personnel involved in train movement, maintenance, or control systems.

FTA: Transit and Public Transportation Employees

The Federal Transit Administration (FTA) enforces DOT drug testing for public transportation workers such as bus drivers, transit operators, mechanics, and maintenance personnel who work for agencies receiving federal funding. These requirements apply to both passenger and non-passenger transit services to ensure safe public transportation systems.

PHMSA: Pipeline and Hazardous Materials Workers

The Pipeline and Hazardous Materials Safety Administration (PHMSA) oversees DOT drug testing for employees involved in pipeline operations and the transportation of hazardous materials. Safety-sensitive roles include pipeline controllers, operators, emergency response personnel, and workers responsible for maintaining or monitoring pipeline systems.

USCG: Merchant Mariners & Maritime Crew

The U.S. Coast Guard (USCG) requires DOT drug testing for merchant mariners and maritime crew members who perform safety-sensitive duties aboard vessels. This includes captains, mates, engineers, deckhands,

What Drugs Are Tested in a DOT Drug Screen?

Health Street's DOT drug test screens for substances required by federal regulations, including:

  • Marijuana (THC)
  • Cocaine
  • Amphetamines
  • Methamphetamines
  • Opiates (heroin, morphine, codeine)
  • Semi-synthetic opioids:
    • Hydrocodone
    • Hydromorphone
    • Oxycodone
    • Oxymorphone
  • Phencyclidine (PCP)
  • MDMA (Ecstasy)

All non-negative results are confirmed using GC/MS testing and reviewed by an MRO for accuracy.

How Long Does a DOT Drug Test Take?

Turnaround times are based on standard DOT testing procedures, HHS-certified laboratory processing, and Medical Review Officer (MRO) verification requirements established by federal DOT regulations. While exact timing may vary slightly depending on the laboratory and individual case, most results are reported within the following timeframes:

  • Negative results: Typically available within 1 business day after the laboratory receives the specimen
  • Confirmed (non-negative) results: Usually completed within 3–5 business days, as confirmatory testing and MRO review are required
  • All DOT drug test results are reviewed, verified, and officially released by a certified Medical Review Officer (MRO) before reporting

Once finalized, results are delivered through a secure online portal, allowing employers and individuals to access documentation quickly while maintaining compliance and proper recordkeeping.

Frequently Asked Questions

Below are the most commonly asked questions about this product in our call center.

How long does it take for a DOT drug test to come back?

The DOT pre-employment drug test time frame is fast. Negative urine specimens typically come back the next business day, but in some cases can take longer. If the drug screening is not negative, further testing is required. Our lab will run a GC/MS confirmation test on all non-negatives urine samples, which provides an official, quantitative measurement of the level of drugs in the system. Confirmation testing takes approximately 3-5 days.

Why is my DOT drug test taking so long?

Most negative DOT drug test results are returned within one business day. However, delays can happen for a few reasons. If the test is non-negative, it must go through a more detailed GC/MS confirmation process, which can take an additional 3–5 days. Delays can also occur because of issues like incomplete paperwork, specimen temperature concerns, or if the Medical Review Officer (MRO) needs to contact the employee for more information, such as verifying prescriptions.

Who needs a DOT drug screen?

All transportation industry modalities require DOT drug screening for various purposes. For example, trucking companies must ensure that safety sensitive positions, such as drivers, are tested on a pre-employment and random basis, as well as for post-accident, reasonable suspicion, return-to-duty, and follow-up testing when indicated.

Who is responsible for paying for the test?

DOT regulations are silent about who is responsible for paying for drug testing. Typically, the drug testing for trucking companies is covered by the company, but not always. However, if a driver fails a test, smaller companies are much less likely to pay for a driver's return-to-duty and follow-up testing, putting the onus on the driver to pay for the tests.

Large fleets, on the other hand, often pay for all logistics drug testing, including return-to-duty and follow-up testing, especially if the drivers are unionized.

Is return-to-duty testing required?

Return to duty tests are only required if a person has failed a test, sought out the counsel of a Substance Abuse Professional, and was subsequently cleared to return to duty by the SAP. DOT regulations require trucking firms to ensure that drivers who are subject to the return to duty process and follow up testing are complying with those testing protocols, including passing all tests as outlined by the Substance Abuse Professional (SAP).

Who reviews the drug test results?

Drug tests are reviewed by Medical Review Officers (MRO). An MRO signature is required on all valid DOT tests, whether positive or negative.

What happens if you fail a DOT physical drug test?

If you fail a DOT drug screen, please review the information provided by the FMCSA (Federal Motor Carrier Safety Administration) to learn more about the next steps. Health Street is available to assist you in scheduling DOT drug screens as required.

What is a DOT Physical?

A DOT physical is a medical exam required for certain workers, like truck drivers, to ensure they meet health and safety standards. It includes vision and hearing tests, a blood test, and a urinalysis. Some packages also combine the DOT physical with a drug test.

Can I take an at-home drug test for DOT-regulated positions?

No. DOT testing is federally regulated, and you cannot take an at-home drug test to fulfill the requirements. Under 49 CFR Part 40, the U.S. Department of Transportation (DOT) only authorizes urine and oral fluid specimens for drug testing, and only when collected under specific procedures at certified collection sites. These specimens must be screened and confirmed at HHS-certified laboratories.

What information does Health Street need so I can send the driver for the test?

To schedule a drug test with Health Street, we will ask for the following information during the registration process:
