This can be done for free at the hospital at the time the child is born. When the form is filed, the father's name is also added to the child's birth certificate. The AOP form can also be completed later, for a small charge, by filing with the Vital Records Central Office.

Reversing an Acknowledgment of Paternity in Louisiana
